Comments:

An unannounced renewal inspection was initiated and concluded on October 25, 2023. There were eight children in care with three staff and the owner. Children in care ranged in age from 12months through three years old. The center allows children to be signed out of care for therapy and signed back in by the therapist working with them. Parent permission for this is included the contract the parent signs with the center. Each child will have its own contract. Children and staff records were reviewed. Children were observed in teacher and self-directed activities. Emergency evacuation documentation, first aid kit, emergency equipment, annual inspections, required postings and transportation were reviewed. Violations regarding staff records, background checks, child's record and playground equipment were observed, discussed with the owner during the exit interview and appear on the violation notice.

Description:	Based on record review and interview the center failed to ensure that background checks are requested within required time frames. Evidence: 1 - The record for Staff #2 has a documented date of hire as June 10, 2023 and documentation that she resided in North Carolina within the past five years. However, the out of state background check was not requested until October 18, 2023. 2 - The owner stated that the out of state background check was requested on October 18, 2023.

Description:	Based on record review the center failed to ensure that staff obtain tuberculosis (TB) screenings as required. Evidence: 1 - The record for Staff #4 did not contain any documentation of TB screening. a. Staff #4 has a documented dat of hire as September 6, 2023. b. Staff #4 was observed preparing meals for children in care during the inspection. c. The owner stated that Staff #4 is a driver for the center as well as the cook.

Description:	Based on record review and interview the center failed to maintain children's records as required. Evidence: 1 - Upon arrival the record for Child #1 was not available in the center. a. Staff #1 and the owner stated that the owner had taken the child's record out of the center. 2 - There is not a separate contract for Child #2 and Child #3. Rather both children's names are on one contract.
