Comments:
A renewal inspection was initiated and concluded on 10/23/2023.There were 43 children present with 8 staff supervising and 1 administrator. The children were observed during reading time engaged in an art project and observed during outdoor play time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:25 am and concluded at 12:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation and staff interview the licensee did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) and 3 (Use Zones for Equipment) on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing. Where steps are used for accessibility, resilient surfacing is not required.

Evidence: Staff & Director confirmed the following playground equipment did not have the required resilient surfacing. Equipment was sitting on top of a type of artificial turf material without cushioning:
1. Yellow spiral slide - 6' high
2. Simplay 3 blue slide - 28" at highest point
3. Step 2 green & blue slides - 24" at highest point
4. Tire walk around bouncer - 15" at highest point
5. Stacked tire climb - 22" at highest point

Plan of Correction: Center owner is researching cost effective forms of resilient surface to address fall zone resilient surface requirements.

Standard #: 8VAC20-780-550-D
Description: Based on record review and staff interview the center did not implement a monthly practice evacuation drill as required.

Evidence: Staff confirmed the center's evacuation drill sheet did not include documentation of evacuation drills in the months of July, August, & September of 2023.

Plan of Correction: Emergency drills to be completed.

Standard #: 8VAC20-780-570-E
Description: Based on observation and staff interview, the licensee did not ensure that prepared infant formula shall be refrigerated, dated and labeled with the child?s name.

Evidence: Staff confirmed, in the Cuddlebug 2 room, there was a bottle of formula in the refrigerator that was not labeled with child #3's name and was not dated.

Plan of Correction: Retrain staff on proper bottle labeling.
