Comments:
An unannounced monitoring inspection was conducted from 9:00am - 1:00 pm with the director. There were 56 children, ranging from three months to four years of age, present with 13 staff supervising. Infant care was observed, and older children were observed playing with table toys, playing in centers, participating in circle time activities and playing on the playground. Five child records and 12 staff records were reviewed along with the written daily attendance, written allergy list, emergency drill log, fire and health inspections, and four accident/incident reports, seven medications and authorization forms, three allergy care plans and the medication log.

Standard #: 8VAC20-770-60-B
Description: Based on review of 12 staff records, the facility did not have a complete sworn statement for all staff. Evidence: the record for staff #9 had a sworn statement that did not indicate whether or not the staff has lived out of the state of Virginia in the past five years.

Standard #: 8VAC20-780-140-A
Description: (Repeat Violation) Based on a review of seven child records, the facility did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #2 (date of first attendance: 6/27/2022), child #5 (date of first attendance: 7/5/2022 and child #6 (date of first enrollment: 9/6/2022) did not have documentation of a physical examination.

Description: (Repeat Violation) Based on review of twelve staff records, not all records contained all required information. Evidence: the record for staff #1 and staff #6 did not contain their date of employment. The record for staff #8 (date of hire: 6/13/2022) did not have complete documentation that two or more references were checked before employment. References cannot be from relatives.

Description: (Repeat Violation) Based on review of the written attendance record, it was determined that the facility did not maintain a written record of daily attendance, for each group of children, that documents the arrival and departure of children in care as it occurs. Evidence: nine children were observed in the preschool IIA classroom, however only 8 children were included on the attendance sheet.

Description: Based on observation, not all areas and equipment inside and outside were maintained in a clean, safe and operable condition. Evidence: there is a rug with curling corners in the preschool IIB classroom that is a tripping hazard; there are divots/rips/gaps in the artificial turf on both playgrounds which are tripping hazards and diminish the resilient surfacing; there is a ?whale on a spring? with a deteriorating face protection buffer system.

Description: (Repeat Violation) Based on observation, the facility failed to use behavioral guidance that was intended to redirect children to appropriate behavior. Evidence: in the preschool IIA classroom, children were observed running, throwing toys, climbing on, and jumping off of furniture. One child was observed stepping on top of another child (who was laying on the carpet) three times. Staff did not intervene, or redirect the children to appropriate behavior.

Description: Based on observation, and staff interview, the facility failed to post documents immediately upon receipt in a prominent place at each public entrance, as required.
