An unannounced complaint investigation was conducted on October 18, 2023, from 3:30pm-4:10pm regarding forbidden action. Interviews were conducted, video footage was observed and documentation was reviewed, There was not a preponderance of evidence to support a violation, therefore the complaint is deemed not valid. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-780-210-A-3 Complaint related: No Description: REPEAT VIOLATION
Based on record review, the center did not ensure all leader teachers met program leader qualifications.
Evidence:
1) Staff 1 is the lead teacher of the Infant classroom. Staff 1has programmatic experience but not the training or education required.Plan of Correction: We will ensure that ALL of the teachers have the required trainings, and experience by sending them through training that meets or exceeds the requirements for the lead position.
Standard #: 8VAC20-780-340-D Complaint related: No Description: Based on record review, each grouping of children did not have at least one staff member who meets the qualifications of a program leader.
Evidence: In the infant classroom, Staff 1 is the lead teacher but does not meet program leader qualifications. This grouping of children (infant classroom) does not have a qualified program leader on staff.Plan of Correction: We will ensure that ALL of the teachers have the required trainings, and experience by sending them through training that meets or exceeds the requirements for the lead position. This will include the Assistant teachers taking these trainings as well.
