On 10/24/2023 an unannounced inspection was conducted in response to a self-reported incident received on 10/16/2023. Interviews and an inspection of the physical plant were conducted regarding the incident. The information gathered during the inspection resulted in violations. Areas of non-compliance are identified on this Violation Notice.

Standard #: 8VAC20-780-340-F Description: Based on interview, the center failed to ensure children under 10 years of age are always within sight and sound supervision of staff.
Evidence:
Staff 1 and staff 2 stated when getting ready to leave the cafeteria, child 1 (age 3) threw away her lunch and instead of getting in line went into the teacher's lounge and hid under the table. The entrance to the teacher's lounge is in the cafeteria. Staff 1 stated the child was found in about three minutes.Plan of Correction: All staff on site will ensure sight and sound supervision of students. Managers will conduct scheduled and unannounced monitoring to ensure compliance; coaching and training will be provided as needed based on observation and monitoring reports.
