Standard #: 8VAC20-780-240-C
Description: Based on record review, the center failed to include some required elements of new staff orientation training.

EVIDENCE:
New staff orientation for Staff 3 (documented employment date of 6/24/2023), Staff 4 (documented employment date of 8/21/2023), and Staff 5 (documented employment date of 8/21/2023) was missing the following components:
Job responsibilities and to whom they report; the policies and procedures listed in subsection D of this section, 8VAC20780-420 A, and the standards in this chapter that relate to the staff member's responsibilities; the center's playground safety procedures unless the staff member will have no responsibility for playground activities or equipment; recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by ? 63.2-1509 of the Code of Virginia; confidential treatment of personal information about children in care and their families; the center's policies and procedures on the administration of medication; emergency preparedness and response planning for emergencies resulting from a natural disaster or a human-caused event such as violence at a child care facility and the emergency preparedness plan as required by 8VAC20780-550 A through K; prevention of sudden infant death syndrome and use of safe sleep practices; prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children; prevention of and response to emergencies due to food and other allergic reactions including: recognizing the symptoms of an allergic reaction; responding to allergic reactions; preventing exposure to the specific food and other substances to which the child is allergic; and preventing cross contamination; and prevention and control of disease.

Plan of Correction: The Director will begin using the model form on the DOE website to document training and ensure all elements are covered. All staff will have new orientation training, to ensure they have training in all required areas.

Description: Based on record review, the center failed to provide to staff in writing the information listed in 8VAC20-780-420-A (known informally as the Parent Handbook), and other policies outlined in this standard.

EVIDENCE:
Documented new staff orientation for: Staff 3 (documented employment date of 6/24/2023), Staff 4 (documented employment date of 8/21/2023), and Staff 5 (documented employment date of 8/21/2023) was missing the following components: evidence they were given a copy of the parent handbook (documentation states it was covered, but not given a copy; procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun;
Procedures to confirm absence of a child when the child is scheduled to arrive from another program or from an agency responsible for transporting the child to the center; lockdown Procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center; procedures for action in case of lost or missing children, ill or injured children, medical emergencies, and general emergencies; policy for any administration of medication; emergency evacuation, relocation, shelter-in-place, and procedures; and precautions in transporting children, if applicable.

Description: Based on record review, the center failed to provide orientation training in first aid and CPR to new staff.

EVIDENCE:
Staff 3 (documented employment date of 6/24/2023), Staff 4 (documented employment date of 8/21/2023), and Staff 5 (documented employment date of 8/21/2023) have no documentation to show they have had orientation to first aid or CPR.

Plan of Correction: The Director will begin using the model form on the DOE website to document training and ensure all required elements are covered. All staff will have current orientation training, to ensure they have training in all required areas.
