An unannounced monitoring inspection was conducted on 10/26/2023 and concluded with an electronic record review on 10/26/2023. There were 16 children, ages 3 years- 4 years, and 2 staff members providing direct care and supervision. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during self-directed activities, teacher-directed activities, and outdoor time. A total of 5 children?s records, and 2 staff records were reviewed. The inspector arrived for the inspection at approximately 10:30 am and departed at approximately 11:15 am.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence: The record for Child 2 contained a documented start date of 08/21/2023. There was not documentation of a physical examination in the record for Child 2. Staff 1 confirmed there was not documentation of a physical examination in the record for Child 2.Plan of Correction: Provider will obtain information from parent.
