22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was conducted on October 24, 2023. Ten staff were present supervising fifty-four preschool age children. Additional staff were on site for administrative duties. Children were observed playing outside and eating lunch. Five staff and five child records were reviewed. Violations found are noted on the violation notice.

Violations:
Standard #: 8VAC20-780-160-B
Description: Based on review, facility did not ensure 5 of 5 staff have acceptable forms of documentation of tuberculosis screening.

Evidence: The records for staff #1, staff #2, staff # 3, staff # 4 and staff #5 had documentation of tuberculosis screenings that were signed by a RN however it was not a physician, physician's designee or an official of the local health department as required.

Plan of Correction: TB tests will be completed for the named staff by November 30th.

Standard #: 8VAC20-780-550-P
Description: Based on review, facility did not ensure required information is documented when a child sustains an injury.

Evidence: The following was found during record review:
1. The record for child #4 had documentation of 3 injury reports that did not document the date and time parent was notified.
2. The record for child #5 had documentation of 1 injury report that did not document the date and time parent was notified.

Plan of Correction: ? On 10-24-2023, I held a staff meeting to review how to correctly complete the form with all PreK staff, PreK TA's, Office Manager, Extended Day Coordinator, and PreK Resource teacher. Essentially any person in the building who is required to fill out an Injury Report was "staffed" with the correct protocol.
? Please see the attached for the model Injury Report I provided to all staff for future reference.
? In the event that someone unfamiliar with the process is completing the Injury Report, I have displayed the model in the Clinic for reference.
