Technical Assistance:
Discussed in detail the requirements (8VAC20-780-330) for where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.

Comments:
An unannounced monitoring inspection was started in the center on October 24, 2023 from 1:20 PM until 3:20 PM and concluded on October 24, 2023. At the time of entrance, 41 children (age range infants to age 4) were in care with 10 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during rest period and preparing for afternoon routine to include snack and play time.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and staff interviews, the licensee did not ensure each staff member shall submit documentation of a negative tuberculosis screening (TB) 30 days prior to employment and coming in contact with children.

Evidence: Staff #5?s (hire date 7-31-2023) did not have documentation of a TB screening and was observed was observed working during the inspection.

Plan of Correction: The staff member has obtained the TB test

Standard #: 8VAC20-780-40-M
Description: Based on observation and staff interviews, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence: The allergy list in the 2 year old room, toddler room and infant room was not dated.

Plan of Correction: The allergy list in all the classes were dated.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review and staff interviews, the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The attendance sheet for the 3 year old room was not accurate, as 4 children were observed in care and only 3 children were listed on the attendance sheet.

Plan of Correction: The child was added to the sign in sheet. Parents were reminded to sign in and out daily.

Standard #: 8VAC20-780-270-A
Description: Based on observation and staff interviews, the licensee did not ensure that inside areas occupied by children, the temperature shall be maintained no lower than 68?F.

Evidence: During the inspection the thermostat in the large program area and the thermostat in the infant room and older infant room both displayed a temperature reading of 66.

Plan of Correction: the thermostat in both rooms are now set to 68F
