An investigation was initiated from 1:30 pm - 3:00 pm in conjunction with a monitoring inspection to conduct an investigation regarding a self-reported incident involving supervision. Interviews were conducted, documentation, children's records, and staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-340-F Description: Based on interviews it was determined that children under 10 years of age did not always remain in sight and sound supervision of staff. Evidence: On 09/13/2023 two staff were outdoors on the playground with three children. At approximately 5:58 pm the toddler classroom was lining up to come inside from the playground. Child A (Age 20 months) was left outside on the playground. Approximately three minutes later staff heard the child crying on the playground and retrieved the child.
