Comments:
An announced administrative file review was completed today between 10am-11:50am. Staff files were reviewed by a representative from The Department of Education, Office of Child Care Health and Safety, at the YMCA Loudoun County Administrative Office located in Sterling, Virginia. A total of 2 staff files were reviewed for this location.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on staff record review, it was determined that the center did not have documentation of a completed sworn statement or affirmation for a staff member.

Evidence:
Staff #2's file did not contain a completed sworn statement or affirmation form. The staff member's first date of hire was documented as 8/18/2018.

Plan of Correction: A new sworn statement has been signed.

Standard #: 8VAC20-780-160-C
Description: Based on record review, the program did not complete Tuberculosis (TB) screening or testing every two years as required.

Evidence:
1. Staff #2's record contained TB results that were last updated on 3/27/2021.
2. Staff #3's record contained TB results that were last updated on 3/11/2021.

Plan of Correction: Staff has been requested to get updated TB.

Standard #: 8VAC20-780-70
Description: Based on record review, required records were not kept for each staff person.

Evidence:
Staff #3's record was missing two references as to their character and reputation, as well as
competency required prior to the first day of employment.

Plan of Correction: New references have been requested if original cannot be attained. HR has been requested to provide them.

Standard #: 8VAC20-780-190-A
Description: Based on record review and interview, there was no documentation on-file to verify that the
program director assigned to the program meets program director qualifications.

Evidence:
Staff #1 is the assigned program director for the program, their file did not contain documentation to verify that they meet one of the 4 program director qualification options.

Plan of Correction: A new program director will be assigned if updated training cannot be attained for existing director.
