A monitoring inspection was conducted on 10/30/23 from 10:45 am to 12:00 pm. There were 12 children present, with 4 staff supervising. The children were observed engaged in movement activities, arts and learning. The infant classroom was open today. There were no children with allergies or medications. A total of 2 children's records, 3 staff records, attendance and emergency drill records were reviewed.

Standard #: 8VAC20-780-140-A Description: Based on record review, the center did not obtain documentation of a physical exam for each child within 30 days of enrollment.
Evidence:
1. Child # 2's first day of enrollment was 7/10/23.
2. Child # 2's record did not have documentation of a physical exam.Plan of Correction: The child does not attend everyday. We will get the physical exam copy from the parents.
Standard #: 8VAC20-780-60-A Description: Based on records reviewed, the center did not have complete information for each child's record.
Evidence: Child # 1's record was missing one emergency contact's address and complete information for the second emergency contact.Plan of Correction: The information will be collected from the parents.
