Technical Assistance:
Discussed the orientation requirements, the importance of taking attendance as the children arrive and depart the classroom and the allergy list placement. Discussed the agents access into the building to inspect facilities.

Comments:
An unannounced monitoring inspection was conducted on 10/31/2023 from 9:52am to 12:32pm. At the time of entrance, 34 children were in care with 8 staff members present. Children were observed playing in centers, playing outside, coloring, playing with playdough having a group discussion about weighing/measuring pumpkins and eating snack. A selection of (3) staff and (3) children records, allergy action plans, the physical space, evacuation drills, medications and attendance records were reviewed. The site was clean and organized. Bulletin boards and several other areas of the programming space was decorated with artwork completed by the children. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.


Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-780-160-A
Description: Repeat Violation
Based on review of three staff records, the center did not obtain documentation of a
negative tuberculosis screening for each staff member prior to hiring and within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #1 (Date of hire 8/23/2022) had a TB test or screening dated 9/14/2023.

Plan of Correction: Noted for future references
especially for the hiring process for
the new employee.

Standard #: 8VAC20-780-40-J
Description: Based on review of the injury prevention plan, the center's Injury prevention procedures were not updated at least annually based on documentation of injuries and a review of the activities and services.
Evidence:
1. The center's injury prevention plan was last updated 8/2022.

Plan of Correction: Prevention plan reviewed and
revised based on the new accident
report that has been filed recently.

Description: Based on interview, the center did not maintain in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8.
Evidence:
1. In the Fuzzy Bears classroom, the staff could not locate the current dated list of all children's allergies, sensitivities, and dietary restrictions.

Plan of Correction: We found the Children Allergies list
been added to the classroom
emergency preparedness binder.
All the staff are all aware where it is
and everyone has access for the
location for emergency purposes.

Description: Based on observation, staff and children's records were not treated confidentially.
Evidence:
1. The supplemental page from the inspection dated 4/06/2023 was posted in the shooting stars classroom. This page is considered confidential.

Plan of Correction: We removed the supplemental
report and kept it in files
confidentially and it was replaced.

Description: Repeat Violation
Based on record review, three children's records did not have complete information.
Evidence:
1. Child #1's file (start date 8/31/2023) was missing both parent's work phone number.
2. Child #2's file (start date 8/31/2023) was missing both parent's work phone number.
3. Child #3's file ((start date 8/31/2023) was missing both parent's work phone number, one emergency contact address and the 2nd emergency contact name, address and telephone number.

Plan of Correction: We notified parents to update
the emergency contact list on the
power school website.
Parents are informed to fill the form
correctly before the submission.

Standard #: 8VAC20-780-70
Description: Repeat Violation
Based on record review, staff records did not include all required information.
Evidence:
1. Staff #1's file (Date of hire 8/21/2023) did not have the staff person's 2 references, documentation to demonstrate that the individual possesses the education required for the position and orientation.

Plan of Correction: The 2 References have been called,
have been documented and added
to her files.
The copy of the diploma has been
printed and added to her files.

Standard #: 8VAC20-780-80-A
Description: Based on observation, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs for each group of children.
Evidence:
1. In the Shooting stars classroom, the attendance was not taken for 10/30/2023..

Plan of Correction: It was corrected and added to the
monthly meeting agenda as a
reminder to have it with them when
they are transitioning to one place to
another.

Standard #: 8VAC20-780-240-C
Description: Based on review of staff records, one staff member did not complete orientation training no later than seven days of the date of assuming job responsibilities.
Evidence:
1. Staff #1's file (Date of hire 8/21/2023) did not have documentation of completing the required orientation training.

Plan of Correction: The staff member file has been
updated with appropriate
documentation of the mandatory
orientation training completed during
teacher work week August17 and
18, 2023. .

Standard #: 8VAC20-780-240-E
Description: Based on review of staff records, one staff member did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment .
Evidence:
1. Staff #1's file (Date of hire 8/21/2023) did not have documentation of completing the required orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.

Plan of Correction: It was noted for future references.
We need to be sure that it should be
documented within 30 days of hiring.
