Technical Assistance:
Technical assistance was offered related to: attendance, medication forms, staff records and qualifications.

Comments:
A monitoring inspection was conducted on 10/18/23 from 9:55 am to 12:15pm. There were 29 children present, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 children's records, 4 staff records, attendance, emergency drill records and medications were reviewed. The children were observed using the bathroom, doing circle time and going outside. Name to face counts were observed prior to exiting the building.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard #: 22.1-289.011-F
Description: Based on observation, the most recent licensing inspection report was not posted in a conspicuous place on the licensed premises.
Evidence: The most recent licensing inspection report dated 3/7/23 was not posted in the facility.

Plan of Correction: The report which was posted on the bulletin board was missing one page which was reposted on the board during the inspection.

Standard #: 8VAC20-780-70
Description: Based on records review, the center did not maintain documentation of complete reference checks for employees.
Evidence: Staff # 2, Staff # 3 and Staff # 4's references did not include the written results of the check.

Plan of Correction: A signature, date and method of contact were being used to maintain written documentation which was approved by previous licensing. Written documentation will now include responses to the questions being asked.

Standard #: 8VAC20-780-500-B
Description: Based on observation, a foot pedal trash can was not used properly when disposing diapers.
Evidence:
1. Staff # 1 was observed changing disposable pull ups for the children in the bathroom.
2. Staff # 1 did not dispose the soiled pull up after each change. They were being collected on the floor of the bathroom.
3. When the changing process was completed, Staff # 1 was observed throwing some of the dirty pull ups in the foot pedal trash can without by opening the cover by her hand that was holding the pull ups. Staff # 1 did not use the foot pedal to open the trash can.

Plan of Correction: Staff # 1 was written up during the inspection and retrained on how to properly change and dispose of pull ups. We will continue to monitor for compliance.
