Comments:
An unannounced monitoring inspection was conducted on 10/30/2023 from 10:21 am-12:23 pm. There were 34 children supervised by 13 direct care staff and volunteers. The physical plant, 5 children records, 1 staff record, 3 volunteer records, evacuation drills, medications, and policies were inspected. Children were observed participating in storytime, tabletop activities, and playing outside. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-780-240-C
Description: Based on a record review and staff interview, the center did not ensure each staff member had completed orientation training in all required facility-specific topics.

Evidence: The record for Staff #1 (hire date 1-3-2012) did not include information indicating orientation related to recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by ? 63.2-1509 of the Code of Virginia.

Plan of Correction: Staff person did the training but lacked a certificate. She will do another that provides a certificate.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside were not maintained in a clean, safe, and operable condition.

Evidence:
1. On the outdoor playground, there was a large pile of tree branches near the student's bike path causing a potential tripping hazard.
2. In the Dragonflies and Ladybugs classroom, there were push pins located in a drawer accessible to children.
3. In the Spiders classroom, there were push pins, safety pins, screw nails, and a nail clipper in a drawer accessible to children.

Plan of Correction: Tree branches moved back into woods.

Push pins moved out of children's reach.

Safety pins, screw nails, nail clipper moved out of reach.
