Comments:
An unannounced monitoring inspection was conducted on 10/30/2023. There were 41 children, ages 2 years-4 years, 9 staff and 3 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications were observed during the inspection. A total of 4 children?s records and 4 staff records were reviewed. The children were observed during active time on the playground, circle time and craft time. The inspector arrived for the inspection at 10:30am and departed the center at 12:30pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-160-B
Description: Based on the review of 4 staff records, the center failed to ensure that each staff member shall submit acceptable forms of documentation of tuberculosis screening.

Evidence: Staff 3 date of hire was 5/30/2023 and on the day of the inspection the documentation of tuberculosis screening did not include the following: physicians signature

Plan of Correction: Provider will obtain tuberculosis screening signed by physician.

Standard #: 8VAC20-780-330-B
Description: Based on observation and discussion with staff, the center failed to ensure that the required amount of resilient surfacing was maintained throughout the playground.

Evidence: Licensing staff observed approximately 2.5 inches of resilient surfacing around the fall zones of the 3 foot tall blue, red, yellow and green play structure and the swing set located by the chain link fence.

Plan of Correction: Provider will add resilient surfacing to playground.
