Comments:

An unannounced renewal inspection was conducted on-site October 19, 2023. The director was available during the inspection. There were 23 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of children?s records on October 19, 2023, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record of child #3, enrolled 07/31/23, contained documentation of immunizations dated 08/08/23. 2. Staff #5 confirmed the immunizations were late.

Description:	Repeat Violation Based on a review of records and interview on October 19, 2023, the center failed to ensure that each staff record contains all required documentation. Evidence: 1. The record of staff #4, hired 06/15/23, did not contain documentation of two references being checked prior to employment. 2. Staff #5 confirmed the reference checks were not completed.

Description:	Based on a review of staff records and interview on October 19, 2023, the center failed to ensure that staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment. Evidence: 1. The record of staff #1, hired 06/23/23, contained documentation of the Virginia Department of Education sponsored orientation dated 10/09/23.02. Staff #5 confirmed it was completed late.

Description:	Repeat Violation Based on observation and interview on October 19, 2023, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. The two year old classroom had an unlocked drawer under the diaper changing table, accessible to children that contained bleach water and a can of Lysol spray. The child safety lock on the drawer was not locked nor was it securely adhered to the drawer. The label on bleach and the Lysol both state "keep out of the reach of children" and "caution." 2. Staff #5 confirmed the drawer was not locked.

Standard #:	8VAC20-780-500-A
Description:	Based on observation and interview on October 19, 2023, the center failed to ensure that staff wash their hands with soap and running water after a diaper change and before feeding and that children's hands are washed with soap and running water after a diaper change or with disposable wipes before eating. Evidence: 1. Staff #2 in the infant room was observed changing a diaper. The staff member did not wash their hands or the child's hands after the diaper change. The staff member then put the child in a feeding chair and fed the child a finger-food snack. 2. Staff #2 confirmed that she didn't wash her hands or the child's hands.
