Comments:

An unannounced on-site renewal inspection was conducted on 10/30/2023 and a staff record review was conducted on 10/31/2023 at the administrative office. There were 45 children, ages 5 years-11 years, and 6 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. The children were observed arriving to the program, during outdoor large motor play, and having an afternoon snack. A total of 5 children?s records, 5 staff records, 1 medication and 5 applicant/agent records were reviewed. The inspector arrived at the center at 2:40 pm and departed at 3:50 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #:	22.1-289.036-A
Description:	Based on record review and discussion with staff, the center failed to ensure that every applicant for licensure as a child day center shall undergo a background check in accordance with the Code of Virginia prior to issuance of a license and every five years thereafter. Evidence: The most recent criminal record check for Applicant/Agent 5 was dated 09/24/2018; There was not a follow-up criminal record check obtain by 09/24/2023.

Standard #:	8VAC20-770-60-B
Description:	Based on record review, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation. Evidence: The record for Staff 4 contained a documented hire date of 10/09/2023. The record for Staff 4 did not contain a sworn statement or affirmation. Staff 4 was observed working during the 10/30/2023 inspection.

Description:	REPEAT VIOLATION Based on record review, the center failed to obtain documentation that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance. Evidence: The record for Child 2 contained a documented start date of 08/16/2023. There was not documentation of a physical examination in the record for Child 2.

Description:	Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and that documentation shall have been completed within the last 30 calendar days of the date of employment. Evidence: The record for Staff 4 contained a documented hire date of 10/09/2023; there was not a tuberculosis screening available for review during the inspection. Staff 4 was observed working during the 10/30/2023 inspection.

Description:	Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing. Evidence: The most recent tuberculosis screening in the record for Staff 1 was dated 03/07/2019; there was not a follow-up tuberculosis screening available for review which would have been required by 03/07/2021 and 03/07/2023.
