An unannounced renewal inspection was conducted on 09/28/2023 between the hours of 10:30am-2:08pm. 50 children were observed directly supervised by 10 staff. The physical plant, 5 staff records, 4 children?s records, 1 Agent record, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed during story time, cleaning up from snack, and preparing to go outside. The center was celebrating "All about apples" week during which they participated in many apple-themed activities. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Evidence:
1. Agent #1 assumed their role on 04/01/2022. The documentation of the results of a search of the Central Registry in the record for Agent #1 was dated 08/22/2023.Plan of Correction: The Central Registry check results were obtained by the center.

Based on record review, the center did not obtain documentation of immunizations before the child's first day for 1 of 4 children.
Evidence:
1. Child #1 began attending the center on 9/06/22. The immunizations in the record for Child #1 were dated 11/11/22.Plan of Correction: The appropriate documents were obtained and placed in the child record. The center has implemented procedures to ensure documents are obtained within required time frames.
