An unannounced monitoring inspection was conducted on October 23, 2023, from 3:30 p.m. through 4:40 p.m. Upon arrival, there were 45 children, ages 5 through 10, in the care of 6 staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed. Annual review of staff records will be scheduled later.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-510-F Description: Based on review of medication the center failed to have parent authorization to administer medication for two children.
Evidence:
1. The medication and medication paperwork was reviewed for two children.
2. Child #1 and Child #2 did not have documentation of parent authorization to administer prescription medicine.
3. Child #1 no authorization for Epi Pen.
4. Child #2 no authorization for Albuterol.

Standard #: 8VAC20-780-510-G Description: Based on review of medication the center failed to have one Albuterol inhaler with the pharmacy label attached.
Evidence:
1. The medication and medication paperwork was reviewed for two children.
2. Albuterol for Child #2 was did not have the pharmacy label attached to include the name, dosage amount and times to be given.
