An unannounced monitoring inspection was conducted on October 26, 2023, from 11:45 a.m. through 12:30 p.m. Upon arrival, there were 77children, ages 3 months through 4, in the care of 17 staff.
The inspector reviewed compliance in the areas of administration,staffing and supervision. A total of 2 staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22.1-289.035-B-2 Description: Based on staff record review the center failed to obtain a criminal history check prior to one staff member's date of hire.
Evidence:
1. Two staff records were reviewed.
2. Staff #2's criminal history check (fingerprinting) was dated 08/28/2023. Staff #2's date of hire of documented on 0822/2023.

Standard #: 8VAC20-770-60-B Description: Based on staff record review the center failed to obtain a signed sworn statement for one staff member prior to the date of hire.
Evidence:
1. Two staff records were reviewed.
2. Staff #2 did not have documentation of a sworn statement being signed and dated before date of hire.
3. Staff #2's sworn statement was signed and dated 08/27/2023. Staff #2' date of hire was documented 08/22/2023.
