Comments:
An unannounced monitoring inspection was conducted on November 1, 2023. There was a total of 67 children with 12 staff members supervising. Children were observed playing with age appropriate materials and games, group activities, and toileting procedures. The inspector reviewed compliance in the areas of administration, staffing and supervision, children and staff records, special care and emergencies. No medication was on sight at the time of inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on observation and interview, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Repeat Violation.
Evidence:
1. There was no written record of daily attendance that documented the arrival and departure of each child in care as it occurred in 3 classrooms (Infants, Toddlers and Twos).
2. The director confirmed the teachers had not documented the arrival and departure of each child as it occurred.

Plan of Correction: I will train my staff again on documenting the arrival and departure of each child as it occurs.

Violation was corrected during the inspection.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation, the center did not maintain the required supervision ratio of one staff member for every four children for children ages birth up to 16 months.
Evidence:
1. At the time of observation (10:10am) the infant classroom had 10 children with two (2) staff supervising. Staff #7 arrived in the classroom at 10:21am.
2. Staff #6 acknowledged they were over ratio because Staff #7 had taken the garbage out.

Repeat Violation

Plan of Correction: There was no reason to be over ratio because we had two extra teachers. I will discuss this issue with my staff immediately. It will not happen again.
