Comments:
An unannounced subsidy health and safety inspection was initiated on 10/30/2023 and completed on 11/01/2023. At the time of the tour, on 10/30/2023, there were 114 children in care with 18 teaching staff. The children in care ranged in age from 12 weeks - five years.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

One violation was found in the area of CPR certification for staff. This violation is listed on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-790-600-D-1
Description: Based on record review, the vendor failed to ensure that CPR certification obtained by staff includes an in-person competency demonstration.

Evidence:
1. The CPR certification for staff 1 and staff 2 did not contain an in-person competency demonstration.
a. The CPR obtained was from an on-line organization not offering any in-person skills demonstration or testing.

Plan of Correction: Both staff 1 & 2 have registered for an approved CPR/First Aid Course on 11/13/23. Primrose will confirm that future hired employees have a certification approved by subsidy.
