Comments:

A monitoring inspection was conducted on October 31, 2023, from approximately 11:00 AM to 2:40 PM. There were 54 children present in the center, ranging in age from infant to five years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of six child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on review of records and interview, it was determined that out of state background checks were not completed as required. Evidence: The record for staff #2 (employed: 01/24/23) who indicated they lived out of state in the past five years, did not contain documentation that an Out of State Sex Offender Registry search was completed for each state prior to their first day of employment.
Plan of Correction:	OM reviewed VADOE regulations regarding out of state background checks and the Interstate Child Care Background Check Contact List with managers to ensure understanding of expectations with each state. Any missing out of state sex offender searches were completed on the same day of the inspection, 10/31/23.

Description:	Based on a review of children's records, the center did not ensure each record contained additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #4 (enrolled: 02/06/23) did not contain one of the updated immunization records which are required every six months for children under the age of two. 2. A member of management confirmed that an updated immunization record was needed.

Description:	Based on review of records and interview, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening (TB) at the time of employment, prior to coming in contact with children, and within the last 30 calendar days of the date of employment. Evidence: 1. The record for staff #2 (employed: 01/24/23) did not have documentation of the TB screening or test completed within the last 30 calendar days of the date of employment. 2. A member of management confirmed that the screening was not completed within the last 30 calendar days of the date of employment.

Standard #:	8VAC20-780-500-A
Description:	Based on observation, the center did not ensure that staff wash hands with soap and running water before and after eating meals. Evidence: In classroom #3, staff #5 put on gloves to serve milk and water to the children without washing their hands.
Plan of Correction:	Management will reinforce LCG policy and VADOE regulation with all staff regarding handwashing and food handling at next staff meeting schedule on November 17th, 2023. The staff member/classroom involved were retrained on handwashing procedures on the same day of the inspection, 10/31/23.
