Technical Assistance:
Technical Assistance: Staff orientation documentation should contain the date of the training and the staff member?s signature acknowledging the training. Children?s records should be reviewed annually by the parents to confirm if any information has changed and parents should initial their confirmation of review in the record.

Comments:
An unannounced monitoring inspection was conducted on 10/25/23 from 10:30am to 2:00pm with the Director. There were 92 children in care, ranging in age from three-months to five-years-old, supervised by 13 staff. The older children were observed playing in structured centers and on the playground; doing ?dot art?, washing hands, and listening to stories; playing with blocks and reading books; listening to the teacher read a story and talk about emotions; transitioning from ?the Brook? back to class, washing hands, and eating lunch; finishing lunch, washing hands, and getting ready for nap by reading books. Infants were observed napping and having free play on the floor. Lunch served was beef macaroni and cheese, steamed broccoli, pears, and milk. Six child records and four staff records were reviewed. The Center has 15 staff with current certification in CPR and First Aid, and 22 staff trained in Daily Health Observation. Two medications and authorization forms were reviewed, and the center has seven staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent fire inspection on record was dated 9/22/22; the most recent health inspection on record was dated 12/5/22. Description: Based on observation, review of documentation, and interview, the center did not ensure that a written record of daily attendance was maintained for each group of children that documented the arrival and departure of each child in care as it occurred. Evidence: On 10/25/23 at approximately 10:49am, eight infants were observed in the Infant classroom. The attendance record contained documentation of only seven children in care. Interview revealed that the eighth child had arrived at approximately 8:36am.

Evidence: The record for Staff #3, with an employment date of 5/8/23, did not contain documentation of Preservice training and management confirmed that the employee had not finished the training yet.

Evidence: Staff #3 was indicated by management as being the Lead in their classroom and was observed to be alone in their class on 10/25/23. Record review indicated the staff was not qualified to be Lead.
