Comments:
An unannounced on-site renewal inspection was conducted on 10/26/2023. There were 16 children, ages 2 years-5 years, and 2 staff members present during the inspection. The inspector observed the following: circle time, free play and snack. A total of 5 children?s records and 2 staff records, were reviewed. The inspector discussed the following with staff: medication and topicals. The inspector arrived at 9:20 AM and departed at 11 AM.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all the elements as required by the standards.

Evidence:

1. The record for Child # 1 has a diagnosed food allergy and did not have a physician completed food allergy action plan.

Plan of Correction: The food allergy action plan will be requested by the parents and will be obtained.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all areas and equipment of the center were maintained in a clean, safe and operable condition.

Evidence:

Observed in main classroom mounted on a shelf was a pencil sharpener that did not have the cover and the sharpening blades were exposed and accessible to the children.

Plan of Correction: The sharpener will be either removed or the cover will be replaced.

Standard #: 8VAC20-780-510-I
Description: Based on review of medications, the center failed to ensure that all medications shall contain all the required elements.

Evidence:

Medication for child # 1 did not have the required prescription label.

Plan of Correction: The parent will be notified that the prescription label was missing. The center will make sure that a label will be obtained.

Standard #: 8VAC20-780-520-A
Description: Based on review of nonprescription medication and topicals, the center failed to ensure that all nonprescription medication and topicals were not kept or used beyond the expiration date.

Evidence:

1.Child # 1 had a nonprescription Zyrtec that had expired on 8/23.
2.There was a center- kept sunscreen that had expired on 6/23.

Plan of Correction: The sunscreen will be discarded, and the Zyrtec will be returned to the parent.

Standard #: 8VAC20-780-520-B
Description: Based on review of sunscreens, the center failed to ensure that all sunscreens were not accessible to children under the age of 5 years.

Evidence:

There was a can of sunscreen sitting on the counter next to the sink that is used by the children. The counter was approximately 2 feet high, and the sunscreen was accessible to the children.

Plan of Correction: The sunscreen will be stored in a manner that is not accessible to the children.

Description: Based on review of insect repellent, the center failed to ensure that all sunscreens were not accessible to children under the age of 5 years.

Evidence:

There was a can of insect repellent sitting on the counter next to the sink that is used by the children. The counter was approximately 2 feet high, and the insect repellent was accessible to the children.

Plan of Correction: The insect repellent will be stored in a manner that is not accessible to the children.

Standard #: 8VAC20-780-530-A
Description: Based on observation, review of two staff records, and interview, the center failed to ensure that all classrooms had a staff person with First Aid and CPR certification.

Evidence:

Staff # 1 was working alone in the in the main classroom area and the staff?s record did not have documentation of First Aid and CPR Certification.

Plan of Correction: First Aid and CPR will be scheduled for all staff that are required to have the certification.

Description: Based on review of the lock down drills, the center failed to ensure that a lock down drill was completed annually.

Evidence:

There was no documented lock down drill for the 2022.

Plan of Correction: All required drills will be completed.
