Technical Assistance:
Safe storage of staff purses was discussed with the program director.

Comments:
A monitoring inspection was conducted on 10/31/2023. The inspector arrived at 11:00 AM and departed at 12:40 PM. There were 40 children present, ranging in ages from infant to four years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-240-I
Description: Based upon review of staff records, the facility has not ensured that documentation of staff orientation training includes all required training topics.
Evidence:
The documentation of orientation training in the records of staff 2 and 3 did not include documentation of all required training topics.

Plan of Correction: The facility responded with the following:
In the future, the model form will be used when documenting staff orientation training.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
There were numerous hazardous substances to include sanitizing wipes and disinfecting sprays on a window sill adjacent to the infant to the infant diaper changing table. No infants were being changed at the time.

Plan of Correction: The facility responded with the following:
All potentially hazardous substances were moved to the cabinet with the child proof latch during the inspection. Staff will be re-trained in hazardous substance storage.

Standard #: 8VAC20-780-550-P
Description: Based upon review of documentation, the facility has not ensured that there is a written record of children's injuries that includes all required entries.
Evidence:
The facility is using the computer application to notify parents of children's injuries. The documentation does not include the staff present at the time of injury, two signatures (either two staff or the staff and parent) and any future action to prevent recurrence of the injury.

Plan of Correction: The facility responded with the following:
In the future, parents will be notified of a child's injury using the app and the model injury reporting form will also be completed and added to the child's file.

Standard #: 8VAC20-780-560-N
Description: Based upon observation, the facility has not ensured that food is prepared in a sanitary manner.
Evidence:
Staff in the infant room was observed serving lunch from a bowl to four different infants using the same spoon. Infant food was not served from individual bowls using individual spoons.

Plan of Correction: The facility responded with the following:
Staff were immediately given separate bowls and spoons from which to serve each infant individually and in a sanitary manner to not allow for cross contamination. All staff will be re-trained on sanitary serving of food to the children.
