Comments:

An unannounced monitoring inspection was conducted on-site October 24, 2023. The director was available during the inspection. There were 25 children present, ranging in ages from 8 months months to 12 years, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of children's records and interview, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The record of child #4, enrolled 08/14/23, did not contain documentation of immunizations. Staff #6 confirmed the immunizations were not obtained.

Standard #:	8VAC20-780-60-A
Description:	Based on a review of children?s records and interview, the center failed to ensure that each child's record contained the required information. Evidence: 1. The record of child #5 did not contain documentation the of the name, address, and phone number of a second designated person to contact in an emergency if a parent cannot be reached. 2. The record of child #2 did not contain documentation of the first date of attendance. Staff #6 confirmed the first date of attendance was not in the record.

Standard #:	8VAC20-780-240-A
Description:	Based on a review of staff records and interview, the center failed to ensure that staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment. Evidence: The record of staff #2, hired 11/14/22, contained documentation of the Virginia Department of Education orientation dated 03/07/23. Staff #6 confirmed the orientation was late.

Description:	Based on observation and interview, the center failed to ensure the staff-to-children ratio is required whenever children are in care. For children birth to 16 months the staff-to children ratio is one staff to four children. Evidence: Outside on the infant and toddler play area Staff #5 was observed holding an infant and caring for five toddlers. Staff # 3 stated the infant was outside with staff #5 for approximately five minutes. Two staff were needed to maintain the ratio.
