Comments:
An unannounced monitoring inspection was conducted today. At LI arrival there were 12 children with 2 staff present in the preschool area. Another staff member arrived with 4 additional children during the inspection. One additional child arrived during the inspection. Staff and child records were not available today, due to the office being locked and no-one present had a key. Medications were reviewed.
Violations include: no prescription label attached to a medication, authorizations incomplete, supervision of children, ratios for preschool groups, no radio available and no health inspection available. Health inspection is required per the Fairfax County Health Department.
Please send to me by Close of Business (COB) 10/26/23: 2 child records, 2 staff records (MAT certification, 1st aid/CPR certifications, Emergency contacts, Daily Health screening certificates).
Licensing Inspector Marbert and Licensing Administrator Ford were present today from 9:45 AM ? 11:35AM.

Systemic Deficiency
Based on observation children under the age of 10 years of age were not within actual sight and sound supervision.
Evidence:
1. LI arrival, (at approximately 10:00AM) staff #2 was observed sitting on a chair outside of the closed bathroom door, next to the office. Staff stated one child was inside the bathroom.
2. 2 girls were observed at the bathroom door to the Girls bathroom, around the corner, playing with the door. 1 child was in the bathroom, while the 2 other girls kept opening and closing the door to the bathroom. The girls were not within sight and sound super vision of any staff person. Staff #2 confirmed there was one girl inside the bathroom.
3. At approximately 11:05 AM , LI and Staff #2 observed 2 girls the backpack/kitchen area going through a child's backpack, out of sight and sound supervision of any staff member.

Systemic Deficiency
Based on observation, a ratio of 1 staff to 10 children aged 3 years old up to school age was not maintained.
Evidence:
There were 13 children observed in the preschool classroom with one staff person. A Second staff person had left the classroom are to fill water bottles during a Show-N-Tell activity.

Standard #: 8VAC20-780-510-I
Description: Based on a review of medications that the center may administer, providing the medication was dispensed from a pharmacy with a prescription label attached, and the center has obtained written authorization from the parent.
Evidence:
1. Child 1's prescription medication did not have a prescription label attached. (Albuterol)
2. Medication authorization did not contain a parent signature. (MAT authorization form)
3. Authorization for a long term medication did not contain documentation of the effective dates of the authorization (MAT form). The Asthma Action Plan form on file did not have any effective dates listed.
