Comments:
An unannounced renewal inspection was conducted on November 6, 2023 from 10:00am-1:15pm. At the time of entrance, 77 children were in care with 17 supervising staff members. The inspector reviewed five children's records and six staff records. Children were observed during diapering, bottle feeding, listening to music, toy play, handwashing, and playground time. Areas of non-compliance are identified on the violation notice. Repeat violations were found and identified within this report.

The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on November 20, 2023. Renewal applications should be submitted through the online portal available on the public website at childcareva.com

Standard #: 22.1-289.035-A
Description: Based on record review, the center did not update criminal history record reports every five years.
Evidence:
1. Staff #5's record (Date of hire-03/01/2022) did not contain an updated criminal history record check. The last criminal history record was dated 09/14/2018.

Plan of Correction: The staff member has an appointment today to complete a repeat criminal history check.

Standard #: 22.1-289.035-B-4
Description: Based on record review, a copy of the results of a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years was not obtained.
Evidence:
1. A sworn disclosure statement states that Staff #6(Date of hire-07/10/2023) has lived in North Carolina in the preceding five years. Staff #6's record did not contain results of a search of the child abuse and neglect registry for the state of North Carolina.

Plan of Correction: Management will continue to look for the document and resubmit the out of state cps search.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record Start date-07/18/2022) did not contain documentation of an annual update of the child's up to date information.

Plan of Correction: The parents will review the child's record and update the information if needed. The parents will document the annual review.

Description: Repeat Violation

Based on record review and interview, the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. Staff #6's record (Date of hire-07/10/2023) did not contain documentation of the Virginia Department of Education-sponsored orientation course.
2. Staff #6 confirmed that the Virginia Department of Education sponsored orientation course had not been completed within 90 days of employment.

Plan of Correction: Moving forward, management will ensure that all staff complete the department's sponsored orientation course within 90 days of employment. The staff member will complete the course and print a certificate to document the record.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation

Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. In Acceleration A classroom, peeling paint was located on the wall next to the exit door of the classroom.
2. In Imagination B classroom, there was peeling paint the size of a quarter located on the wall in the library area of the classroom.

Plan of Correction: Building maintenance will spot paint the areas in the classrooms with peeling paint.

Description: Based on observation of the playground, the center did not maintain resilient surfacing.
Evidence:
1. On the big playground, the playground structure did not have resilient surfacing of six inches of mulch. The ground was exposed under the slide.

Plan of Correction: Mulch will be ordered today for the playground

Standard #: 8VAC20-780-500-A
Description: Based on observation, children's and staff hands were not washed with soap and running water after a diaper change.
Evidence:
1. Staff #1(Date of hire-10/20/2023) changed two children's diapers and put them down on the floor without washing their hands.
2. Staff #1 did not wash their hands before and after two diaper changes.

Plan of Correction: The staff member will be retrained

Standard #: 8VAC20-780-550-D
Description: Repeat Violation

Based on documentation review and interview, the center did not implement a monthly practice evacuation drill.
Evidence:
1. An evacuation drill was not conducted for the month of May. The documentation log of evacuation drills did not contain an evacuation drill for the month of May.
2. Staff #4 confirmed that an evacuation drill was not conducted for the month of May.

Plan of Correction: Due to management transition, an evacuation drill was not conducted and not logged. Monthly evacuation drills are conducted by the center. To make up for missed evacuation drill, the center will conduct an extra drill during the month.
