Comments:
An unannounced monitoring inspection was conducted on November 8, 2023, from 9:30am-12:00pm. At the time of entrance, ( 65 ) children were in care with (15 ) staff members present. The inspector reviewed 4 children?s records and 4 staff records. Children were observed engaged in group manipulatives, blocks, story time and lunch. Medications were observed. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observation and staff interview, the center has not maintained a dated current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan
Evidence: A allergy list was found posted in each classroom, this list did not contain children's sensitivities and was not dated.

Plan of Correction: Food sensitivity will be included on the allergy list and the list will be dated.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of medications, the center has not maintained a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The center has an allergy action plan for child #1 but it was completed by the parent and does not have the physicians instructions for the steps to be taken in the event of a suspected allergic reaction.

Plan of Correction: The parent will have the physician complete the allergy action plan form and return to the center.
