Comments:
A subsidy health and safety (SHSI) inspection was conducted on September 27, 2023 from 4:00 P.M.-5:00 P.M. There was one child present with one caregiver supervising. The inspector reviewed on child record, one caregiver, and one household members record to ensure documentation was complete. Areas reviewed were administration, physical plant, hazardous materials, posting requirements, and vehicles.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-A
Description: Based on review of the provider records, the provider failed to ensure that background checks were updated every five years.

Evidence:
1. The National Criminal History background check, (fingerprint), in the providers record was dated 09/18/2018. A new one was required by 09/18/2023.
2. The Central Registry Finding in the record was dated 09/25/2018 a new one was required by 09/25/2023.

Plan of Correction: The provider will set up an appointment to obtain the fingerprints, and send off for a new central registry finding. The provider will send evidence to the Licensing Inspector within 10 business days that the background checks have been completed or sent off.

Standard #: 8VAC20-790-250-F
Description: Based on a review of the providers record, the provider failed to obtain annually at least 16 hours of training.

Evidence:
1. The provider only had five hours of annual training for the training year 2022-2023.
2. The provider verified that they had not obtained all the required training hours.

Plan of Correction: The provider will ensure she receives 16 hours of annual training hours for the coming year and place all certificates in her record.

Standard #: 8VAC20-790-270-A
Description: Based on observation, the provider failed to ensure that hazardous materials were inaccessible to children.

Evidence:
1. In the bathroom on the counter there was a container of Good Cleaning Disinfectant wipes sitting on the sink that was accessible to children.
2. In the living room near the desk there was a bottle of Lysol Disinfecting Spray, and a container of Fabuloso Disinfecting Wipes sitting on shelf that was accessible to children.
3. Manufacturer labels stated "keep out of the reach of children," and "hazardous to children and domestic animals."

Plan of Correction: The hazardous materials will be moved to an area that is inaccessible to the children.
