Technical Assistance:
Discussed broken cabinets in front room. Cabinets currently have exposed hold which may cause finger entrapment.

Discussed ensuring the most current version of the allergy list is posted in all classrooms.

Comments:
An unannounced renewal inspection was conducted today from 9:40am-11:10am. There were 34 children directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, tracing the letter "z", playing in dramatic play, making art projects. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.011-F
Description: Based on observation, the findings of the most recent licensing inspection were not posted in the facility.
Evidence:
The findings from the inspection conducted on 8/21/2023 were not posted in the facility on the date of inspection.

Plan of Correction: 1. Post all violations the day they're provided.

Standard #: 8VAC20-780-210-A
Description: REPEAT VIOLATION
Based on observation, staff statement, and record review, one staff member was operating as a program lead without documentation to support that they are qualified in that position.
Evidence:
On the date of inspection Staff 1 was operating as the program lead in the three year old classroom. Staff 1's record did not contain documentation that they have met the qualifications of a program leader.

Plan of Correction: Continue to work on getting staff qualified whether it be promotion/new lead teacher hires.

Standard #: 8VAC20-780-280-B
Description: REPEAT VIOLATION
Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. An unlocked cabinet in the four year old classroom contained: disinfectant wipes, spray cleaner, aerosol disinfectant, and spray air freshener.
2. An unlocked cabinet in the toddler year old classroom contained: disinfectant wipes, spray cleaner, and a spray bottle of bleach and water.
3. An unlocked cabinet in the three year old classroom contained: spray cleaner.

Plan of Correction: 1. Make sure to always check cabinets are locked during walk throughs. 2. Replace broken/damaged locks.

Standard #: 8VAC20-780-510-E
Description: Based on a review of medication, the center did not follow its procedures to administer medication.
Evidence:
1. Child 6 has a written authorization to administer Diphenhydramine, however the medication present for the child on the date of inspection was Diphenhydramine and Phenylephrine.
2. Child 7 had a written authorization to administer Acetaminophen, however the medication present for the child was Ibuprofen.

Plan of Correction: 1. Send home incorrect medication. 2. Have parent's fill out new form with correct medication name.
