A self reported other monitoring inspection was initiated on 10/30/23 and concluded on 10/30/23. The inspector reviewed compliance in the area of staffing and supervision. The inspection started at 3:00pm and concluded at 3:30pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Description: Based on discussions the center failed to ensure the care, protection, and guidance of a child as required.
Evidence:
On 10/30/23 at approximately 11:47am a child was left outside on the playground by themself when the rest of the class and staff went inside. When the head count was completed inside, the staff realized a child was unaccounted for. Staff went back outside where the child was sitting in the tree playground structure. The approximate time the child was unattended was 7 minutes.
Staff did not ensure the care, protection, and guidance of this child as required.Plan of Correction: Staff provided their explanations of the event and licensing was contacted as required, Staff have been provided a refresher on playground safety /supervision and the site will also be implementing a form for posting the number of children on the playground at all times.
