An unannounced on-site non mandated inspection was conducted on 10/30/2023. There were 8 children, ages 4 years-8 years, and 2 staff members present during the inspection. The inspector observed the following: arrival, free choice activities. A total of 5 children?s records, were reviewed. The inspector arrived at 6 AM and departed at 7 AM. Description:	Based on record review, the center failed to ensure that each report of immunization and physical examination shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: 1. The immunization report and physical exam in the record for Child # 2 was not dated by a physician, his designee, or an official of a local health department.

Description:	8th REPEAT VIOLATION Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all the elements as required by the standards. Evidence: 1. The record for Child # 1 have the up-to-date information in the child?s record annually as required by 8 VAC20-780-420 E 3. 2. The record for Child #3 has a diagnosed food allergy and did not have a Food Action Allergy Plan completed by the Physician. 3. The record for Child # 5 did not include the following information: the addresses for the required two designated people to call in an emergency if a parent cannot be reached.

Description:	4th REPEAT VIOLATION Based on observation, the center failed to ensure that all chemicals were locked using a safe locking method that prevents access by children. Evidence: 1. In the preschool room sitting on the desk was a Febreze aerosol can. Children were observed in the room and the can was accessible to the children.
