Comments:
An unannounced monitoring inspection was conducted on 10/31/2023 from 3:35 pm to 4:15 pm, and staff records were reviewed on 11/1/2023 and 11/2/2023 at the main office. There were fourteen children in care, ages six years to ten years, in one group under the care of two staff. The Licensing Inspector reviewed two children?s records, three medications and associated records, two staff records, and other program records. The children were observed doing quiet activities and getting ready for snack time.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to have some required information in a child?s file.

EVIDENCE:
The registration for Child 1 was missing the physician?s phone number.

Plan of Correction: Staff will obtain this information and add it to the child's file.

Standard #: 8VAC20-780-340-D
Description: Based on record review and interviews, the center failed to have a qualified program leader or director in each group of children.

EVIDENCE:
Neither of the two staff on the site were qualified to be program leaders or directors. The records for Staff 1 and 2 had no proof of education. Staff 2 had insufficient training to qualify as a program lead. This was confirmed by Staff 1 and 2 during the onsite inspection, and by Staff 3 during the offsite staff record review.

Plan of Correction: Proof of education, experience, and training will be added to the staff file so that program leader qualifications are documented.
