Comments:
An unannounced monitoring inspection was conducted today November 9, 2023 to determine the centers compliance with licensing standards. The licensing inspector was on site from approximately 2:15 pm until 4:00 pm. The Director assisted with the inspection. There were 24 children present, ranging in ages from 16 months to five years, with five staff supervising. Upon the inspector arrival, children were napping. Later children were observed having snack, coloring, listening to a story. Children also worked on balance skills as they walked across the sensory steppingstones.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. The inspector interacted with staff and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on the review of five staff records, the center did not obtain documentation of a negative tuberculosis screening for one staff member within the required time frame.

Evidence:

1. The record for Staff #1 (employment 09/25/2023) contained documentation of a negative tuberculosis screening dated 07/17/2022.
2.Tuberculosis screenings are required to be obtained at the time of employment and have been completed within the last 30 calendar days prior to beginning employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Per the director: Going forward I will have staff provided current TB screening for their records.

Standard #: 8VAC20-780-160-C
Description: Based on review of five staff records, the center did not ensure that one staff obtained and submitted the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening.

Evidence:

The record of staff #3 contained documentation of a tuberculosis screening dated 07/08/2021, exceeding two years.

Plan of Correction: Per the director: I will have staff update their TB screening.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean safe and operable condition.

Evidence:

Multiple seams of the artificial turf mat coving the ground of the playground were curled upward creating a tripping hazard.

Plan of Correction: Per the director: I will add additional stakes to ensure the edges are flush and not a tripping hazard.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not ensure children?s hands were washed with soap and running water after toileting.

Evidence:

1. Staff #3 did not wash the hands of child A and child B after diapering each child.
2. Staff #3 did not her hands with soap and running water before and after a diaper change.

Plan of Correction: Per the director: I will review hand washing procedures with all staff.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use.

Evidence:

1. After changing the diaper of child A, staff #3 was observed changing the diaper of child B. Staff #3 did not clean the diaper mat surface with soap and water and sanitize after each use.
2. The diaper pail in the toddler room was not a foot-operated or a system that could be used in such a way that neither the staff member?s hand nor the soiled diaper touches the exterior. Staff #4 stated she used her hand to open the pain to deposit soiled diapers.

Plan of Correction: Per the director: I will purchase a new diaper pail that is hands free. I will review proper diapering procedures with staff.
