A complaint was received regarding concerns in the areas of physical plant, administration, staff qualification and training, staffing and supervision, programs, special care provisions and emergencies and special services.
Interviews, observation and a review or recordkeeping were conducted and part of the complaint involving staffing and supervision and special services were determined to be valid. The areas of non compliance is outlined on the violation notice.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: Based on interviews and a review of documentation, it was determined that when staff were supervising children, they did not always ensure their care, protection, and guidance.
Evidence:
1. On 8/31/23 at approximately 8:30 am child B, 13 months of age pulled a hot cup of coffee onto their chest. The hot coffee was placed on a lower shelf in the infant room by staff C. The child?s chest turned red and the child cried in discomfort. At the end of the day there remained some red discoloration on the chest that subsided over two days.
2. On 9/7/23 at approximately 12 noon, staff A and B fed child A cheese although the child has a diagnosed food allergy to diary.Plan of Correction: An intensive plan of correction will be provided at a later date.
Standard #: 8VAC20-780-560-P Complaint related: Yes Description: Based on interviews and a review of recordkeeping, it was determined that a child with a diagnosed food allergy was served food identified in the written allergy care plan.
Evidence:
Child A, 2 years of age has a diagnosed food allergy to diary. On 9/7/23, staff A and B served the child a cheeseburger for lunch at approximately 12 noon. The center documented the meal on the center?s application software, which was viewed by child A?s parent at approximately 12:20 pm. The parent contacted the center to remind them that child A was allergic to the cheese on the cheeseburger. Interviews revealed that the center monitored the child?s health the remainder of the day and their appeared to be no apparent adverse reaction as a result of the child being exposed to the cheese.Plan of Correction: An intensive plan of correction will be provided at a later date.
