Technical Assistance:
First Aid/CPR certification of the bus driver was discussed with the program director.

Comments:
A monitoring inspection was conducted on 11/09/2023. The inspector arrived at 10:30 AM and departed at 1:05 PM. There were 8 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-60-A
Description: Based upon review of children's records, the facility has not ensured that each child's record includes the name, address, telephone number and employment information for the parents or guardians.
Evidence:
The record provided for child 2 did not include any information regarding the father. N/A was written on the enrollment form and no additional information was in the record.

Plan of Correction: The facility responded with the following:
Facility administrator will speak with the mother to obtain additional information regarding the child's father. Al children's records will be checked to ensure that required information is complete.

Standard #: 8VAC20-780-510-E
Description: Based upon review of the center's medication policies, medicine stored and documentation, the facility has not ensured that the center's procedures for administering medication include the duration of the parent's authorization for medication, provided that it shall expire or be renewed after ten working days.
Evidence:
1. There was over the counter cetirizine stored for administration to child 1. The written parent authorization for the cetirizine to be administered expired on 7/14/2023 and was not renewed.
2. There was albuterol stored for potential administration to child2. The written parent authorization for the albuterol to be administered expired on 7/1/2023.

Plan of Correction: The facility responded with the following:
Parents will be contacted to obtain required authorization updates. A system will be put into place and reviewed with MAT trained staff to routinely check the forms authorizing medication to ensure they are up to date.
