An unannounced Monitoring Inspection took place on November 13, 2023 between the hours of approximately 11:05 a.m. and 1:35 p.m. There were 10 classrooms observed with a total of 115 children with 17 staff within the supervision guidelines. The children were observed napping, eating lunch (vegetarian sloppy joes, cheesy zucchini, applesauce and bread), playing with blocks and magnets and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, and fire drill log was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center failed to obtain all information for children?s records.
Evidence:
1. Child #3?s (start date: 12/05/2022) record did not contain documentation of viewing proof of the child?s identity and age.
2. Child #5?s record did not contain documentation of confirmation of up-to-date information. The most recent update was dated 07/05/2022.
