Technical Assistance:
Measurements were taken and due to modifications of the center the capacity will be changed. Awaiting approval from the building official.

Comments:
An unannounced renewal inspection was conducted today from approximately 10:40am-12:50pm. There were 20 children in the direct care and supervision of 7 staff. A sample of 3 children?s records, and 3 staff records were reviewed. The children were observed walking outside, napping, and getting ready to eat. Two new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Standard #: 8VAC20-780-60-A
Description: Based on record review, the center did not maintain a complete record for each child enrolled.

Evidence:
1) The record of Child A was missing an area for "nick name" and the mother's work phone number.
2) The record of Child B was missing an area for "nick name" and 1 of 2 required emergency contact addresses.
3) The record of Child C was missing an area for "nick name", 1 of 2 required emergency contacts, employer number of both parents and the name and phone number of the physician.

Evidence:
1) The record of Staff B was missing the address of an emergency contact.

1) Staff A stated that she changed the pull up of a child inside the classroom and without a non-absorbent surface.
2) The pull up was located in the trashcan located in the classroom.
