A monitoring inspection was conducted on 11/13/2023 between the hours of approximately 12:20 pm and 2:30 pm. There were 48 children present, ranging in ages from 8 months and 5 years, with 7 staff directly supervising the children. Children were observed during their designated rest period.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-M Description: Based on documentation review, allergy plans were not dated or kept confidential in each room or area where children are present as required.
Evidence:
1) Allergy plans were not available for review in the following rooms: Infants, Toddlers, 2 year-olds, and 3 year-olds.
2) The allergy list available in the 4/5 year-olds room was not dated nor covered to maintain the information confidential.Plan of Correction: Allergy plans are now available in all 5 classrooms and dated.
Standard #: 8VAC20-780-60-A Description: Based on record review, children's records did not contain all required information.
Evidence:
10 The record for Staff #1 did not contain the work phone number of each parent who has custody.Plan of Correction: The child's parent was contacted on 11/15 and has provided with their work number.
Standard #: 8VAC20-780-240-C Description: Based on record review, orientation training did not include all required topics.
Evidence:
1) The record for Staff #4 did not contain documentation of having completed training in Safe sleep practices and Shaken baby syndrome/Head trauma.Plan of Correction: The new documentation is now updated.
Standard #: 8VAC20-780-245-J-3 Description: Based on documentation review, child for whom emergency medications have been prescribed were not always under the care of a staff member who is MAT trained.
Evidence:
1) Child #1 has emergency medication in the center. Staff assigned to Child #1's class are not MAT trained.Plan of Correction: Staff assigned to Chiild #1 will have MAT training no later than 12/11/2023.
