Provided consultation on standard:
1) 40-N: The center shall develop written playground safety procedures that shall include: method of maintaining resilient surface.
2) 270: Areas and equipment of the center inside and outside shall be maintained in a clean, safe and operable condition. Rust is an indicator of deterioration, and can compromise the integrity of the item.
3) 500B-3c: A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing. Children under age three cannot be changed standing up in the restroom before/after using the potty. If the diaper/pull up is wet, they must be changed on the changing table.

An unannounced monitoring inspection was conducted from 10:30 am - 1:30 pm with the center assistant director. There were 88 children, ranging from five months to four years of age, present with 15 staff supervising. Infant care was observed and older children were observed playing outside on the playground, participating in structured learning centers, and eating lunch. Lunch served today: chicken and cheese wrap on a whole wheat tortilla, corn, peaches and milk. Six child records and six staff records were reviewed, along with the written daily attendance, written allergy list, emergency drill log, fire and health inspections, and seven medications and authorization forms.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Tuesday, 11/21/2023. Standard #: 22.1-289.035-A Description: Based on review of six staff records, it was determined that not all staff had repeat background checks every five years as required. Evidence: The record for staff #6 had Central Registry search results dated: 9/5/2018. The record for staff #1 had a criminal history (fingerprints) dated: 8/21/2018, The record for staff #4 had a criminal history (fingerprints) dated: 8/13/2018 and the record for staff #6 had a criminal history (fingerprints) dated: 8/21/2018. Plan of Correction: Staff has been to their appointments.
Standard #: 8VAC20-780-130-A Description: Based on a review of six child records, the facility did not obtain documentation that each child has received immunizations prior to the child's first attendance. Evidence: the record for child #4 (date of first attendance: 1/16/2023) did not have documentation of immunizations. Plan of Correction: Corrected onsite of visit
Standard #: 8VAC20-780-140-A Description: Based on a review of six child records, the facility did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #4 (date of first attendance: 1/16/2023) did not have documentation of a physical examination. Plan of Correction: Parents have been updated. Forms requested.
Standard #: 8VAC20-780-160-C Description: Based on review of six staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #1 had a TB test/screening dated: 7/18/2021. Plan of Correction: Teacher received TB on 10/31/23.
Standard #: 8VAC20-780-510-E Description: Based on review of seven medications and authorizations, it was determined that the facility failed to follow their own medication procedures as required. Long term prescription drug use and over-the-counter medication may be allowed with written authorization from the child's physician and parent. Evidence: the record for child #7 did not contain written authorization from the child's parent for long term medication observed on site. Plan of Correction: Correct documentation was given to parent. Will receive asap.
