Comments:
An unannounced monitoring inspection was conducted on 10/24/2023 from 10:19 am to 12:11 pm. There were two children in care, ages three years to four years, in one group under the care of two staff. The Licensing Inspector reviewed two children?s records, two staff records, and other program records. The children were observed playing and having lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on observation, the center failed to post the most recent results from an inspection.

Standard #: 8VAC20-780-40-E
Description: Based on observation and document review, the center failed to follow their playground safety policy.

EVIDENCE:

The New Beginnings Learning Center Playground Safety policy references checking the surface under playground equipment and keeping a surface depth of at least 9 inches. The area at the exit of the pink slide had approximately 2 inches of mulch surfacing. The area at the exit of the red slide had approximately 3.5 inches of mulch surfacing. 9 inches is required.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION

Based on observation, areas and equipment inside and outside of the center were not maintained in clean, safe, operable condition.

EVIDENCE:
1. A climbing toy with a slide on the playground was missing stairs, leaving approximately 2 feet where a child could fall off the platform.

2. The front steps leading up to the center are rotting away and have several splintered sections. The railing on the porch at the top of the steps has loose vertical slats with nails sticking out. Some of the steps are splitting apart and are a trip hazard.

3. Some of the paint in the front room of the center is peeling and in reach of children.

4. The back door to the center, which leads out to the playground, has metal coming loose and is a pinch hazard.

5. In the blue-gray classroom panels of wallpaper are peeling off the bottom section of the wall. The peeling area, within reach of children, was approximately five feet wide and five feet high.

Description: REPEAT VIOLATION

Based on observation around climbing equipment, the center failed to ensure all immediate fall zones had the required amount of resilient surfacing.

EVIDENCE:
1. The area at the exit of the pink slide had approximately 2 inches of mulch surfacing. The area at the exit of the red slide had approximately 3.5 inches of mulch surfacing. 9 inches is required.

2. Six feet four inches is required to be in front of the pink slide and six feet three inches is required to be in front of the red slide. They don?t have separate use zones and the slide chutes face each other diagonally.

3. The pink slide structure was 12 inches away from the raised boundary of the playground and six feet is required.

4. The red slide structure was two feet away from the raised boundary of the playground and six feet is required.
