Comments:
A complaint inspection was conducted on-site on November 16, 2023 from 11:05 A.M.-2:40 P.M., telephone interviews were conducted on September 19, 2023, and the inspection concluded on November 16, 2023. A complaint was received in the Office of Child Care Health & Safety (Valley), on September 19, 2023 in regards to staffing, supervision, and programs. The inspector reviewed compliance in the areas of administration, staff records, staffing, and supervision. Eight staff records were reviewed/updated, five staff were interviewed, and classroom observations conducted. The information gathered during the investigation does support part of the allegation, so the complaint is determined to be "valid".

Information gathered during the inspection determines non-compliance with applicable standards or lawn and violations were documented on the violation notice issued to the program.

Complaint related: Yes
Description: Based on interview and record review, the center failed to ensure that at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence:
1. Staff #3 (DOH 09/12/2023) and Staff #4 (DOH 10/16/2023) were working in the classroom together. The staff records revealed both Staff #3 and Staff #4 were hired as Assistant Teachers and did not have the experience or qualifications to be program leaders.

Plan of Correction: We will begin to look for more people with the 6 months experience and complete the 24 hours training within the 30 days.

Complaint related: Yes
Description: Based on interviews, the center failed to ensure that an additional staff member shall always be available on-site to offer immediate assistance and shall be able to summon the additional staff member without leaving the room or area of the sleeping or resting children.

Evidence:
1. Staff interviews revealed that three of the five staff were interviewed and confirmed that during lunch/rest/nap time there is not an additional staff member on-site to offer help if they needed immediate assistance.

Plan of Correction: We are in the process of hiring two extra PT administrative people that can also step into the classroom or help throughout the day.
