An unannounced inspection to investigate a self reported incident was initiated by phone on 9/13/2023 and completed on site on October 4, 2023. The incident was regarding supervision and injury reporting. Documentation was reviewed, staff were interviewed, and observations were made at the center.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the self reported incident was determined to be not valid. There were no violations resulting from this inspection.
Time of inspection: 12:30 p.m. to 1:15p.m.
