Comments:
An investigation was initiated from 12:15 pm - 2:45 in conjunction with a monitoring inspection to investigate a complaint received by the department involving behavioral guidance. Interviews were conducted, video footage, documentation, staff records, and child records were reviewed. The complaint regarding behavioral guidance was determined to be valid. Standard #: 8VAC20-780-400-B
Complaint related: Yes
Description: Based on interviews and review of video footage it was determined that behavioral guidance was not always constructive in nature, age and stage appropriate, or intended to redirect children to appropriate behavior. Evidence:

1.) On 10/27/2023 at 9:45 am the children in the two's classroom were observed sitting on the carpet with Staff A and Staff B and were participating in circle time activities. Child A (age two) was observed attempting to leave the group on the carpet. Staff A is observed grabbing child A by the leg and drags the child back toward their lap. When the child attempts to get up again, Staff A grabs the child by the arm and begins to drag the child back towards their lap. Child A then falls back and hits their head on the ground. Staff A then places child A in their lap.

2.) At 9:47 am during the same circle time activity child B (age two) retrieves a plastic toy horse. Staff B aggressively takes the toy from child B and throws the toy across the room.

3.) At 9:50 am the children in the two year old room begin to retrieve their coats from behind the classroom changing table. While the children are putting on their coats, child B climbs onto the changing table. It is reported that one of the staff yelled loudly in a harsh voice tone at child B. Staff A then proceeds to the changing table to retrieve child B from the changing table. The interaction between staff A and child B is not seen because the changing table was blocking the view of the camera.

Plan of Correction: 1.) Staff A was requested to review camera footage. Staff A was placed on a Final Written Warning in regard to the violation related concerns of Standard 8VA20-780(6)-400-B. Staff A will receive continuous monitoring and documented coaching. Staff A will also receive refresher training about teacher interactions focusing on the C.L.A.S.S. lense, specifically the Teacher Sensitivity Domain.
2.) Staff B was requested to review camera footage. Staff B was placed on a Written Warning regarding violation related concerns of Standard 8VAc20-780(6)-400-B. Staff B will also receive refresher training about teacher interactions focusing on the C.L.A.S.S. lense, specifically the Teacher Sensitivity Domain.
3.) Staff A and Staff B were spoken to about the tone of their voice. Ongoing coaching about appropriate practices will take place. Refresher training regarding appropriate practices and in regard to Standard 8VAac20-780-(6)-400-B. Coaching and monitoring to both staff will be given.

Complaint related: No
Description: Based on review of video footage it was determined that staff failed to follow procedures with regards to handwashing. Evidence: Staff A is observed wiping a child's nose and does not wash their hands after cleaning the child's nose. Staff A is observed changing a child's diaper and does not wash their hands after the diaper change.

Plan of Correction: Staff A received refresher training for appropriate handwashing practices. Staff A will continue to be monitored and coached in regard to Standard 8VAC20-780-(7)-500-A.
