Comments:
A monitoring inspection was initiated on 10/26/2023 and concluded on 10/30/2023. There were 24 children present, ranging in ages from 3 years to 4 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on a review of records and interview, the center did not request an out-of-state criminal name check for each employee hired who has resided in any other state in the preceding five years.

Evidence:
1. The record of Staff 1 (DOH 07/20/2023) did not contain documentation that an out-of-state child abuse and neglect search was completed. Staff 1 identified living in one other state in the past 5 years on the staff's Sworn Disclosure Statement.

Description: Based on observation, the center failed to ensure that medications shall not be stored in purses that are accessible to children.

Evidence:
1. During observation, a staff members purse was located underneath the desk with a bottle of Advil located inside the purse.
