Comments:
A monitoring inspection was conducted on 11/13 and 11/14/2023 in response to a self reported incident where a child was served a food containing a known allergen and was subsequently treated for an allergic reaction.

Staff interviews were conducted, 2 staff files and 1 child's file was reviewed. Areas of non compliance are identified in this report.

Violations:
Standard #: 8VAC20-780-560-P
Description: Based on interviews and record review, a child with a diagnosed food allergy was served food identified in the written care plan.
Evidence:
1. On 11/1/2023 Staff 1 served Child 1 pudding containing dairy, which is a documented allergen for the child.
2. Child 1's parent was notified by telephone within fifteen minutes of ingestion when the child was noted to be flushed, crying, and throwing up.
3. Child 1's parent arrived at the center and administered emergency medication to Child 1, and took the child to be medically evaluated.

Plan of Correction: To address the violation, we have taken the following corrective actions:

1. Employee Training: On 11/2/23, all staff members involved with the incident underwent immediate training on allergy prevention and policies to ensure heightened awareness and adherence.

2. Use of Red Plates: We have introduced the use of red plates specifically for children with allergies. Effective immediately, an allergy student's meal is plated in the kitchen by the chef on red plates to facilitate easy recognition by all staff members.

3. Comprehensive Training Session: On November 14th, 2023 an intensive training session was conducted for the entire faculty, including leadership and the chef. This training covered in-depth aspects of allergy prevention, procedures, and policies to enhance overall understanding and compliance.

These measures have been promptly implemented to prevent the recurrence of similar incidents and to safeguard the safety and well-being of the children under our supervision.
