Comments:
An unannounced monitoring inspection was conducted today from 10:20am-12:15pm. There were 82 children (infants-5yrs.) directly supervised by 22 staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, circle time, reading books, playing outside, and napping. There was an adequate supply of materials for the children. The center was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy.

Evidence:
Child #6's record did not include a written care plan with instructions from the physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: Reached out to parents to get the written care plan from physician.

Standard #: 8VAC20-780-560-G
Description: Based on observation, not all bottles were dated as required.

Evidence:
In the infant room, three bottles were observed in the classroom's refrigerator labeled with the children's name, but they were not dated.

Plan of Correction: Dates were added to bottles during visit. Reminder to teachers to date all bottles.
